What to include — and what to leave off
Include
- Credentials and current licensure/certification status up front.
- Case mix, annual volume, and supervision model — concrete numbers.
- Subspecialty strengths (cardiac, OB, peds, regional, pain).
- ACLS/BLS/PALS, board/CPC status, and any fellowships.
- A short, specific summary tailored to the role you want.
Leave off
- SSN, full DOB, photo, or other sensitive personal data.
- Unexplained gaps with no context, or vague “team player” filler.
- Outdated certifications presented as current.
- References on the document — “available on request” is enough.
