Schedule: Monday-Friday, 10hr shifts, Fridays a must o Is call required? No

• Types of Cases: PEDS, Adult ortho, GYN/ONC, Urology, Complex Spine, Ophthalmology, Plastics, ECT; Specific PEDS – Cath lab, Peds ENT, Peds general surgery, Peds urology, Peds BMT, Peds Oculoplastic. Providers also expected to work in Donor Care unit. o Experience with all Peds surgeries (general, neurology, neuro-spine, ENT, offsite MRI, Ophthalmology, GI, and ortho) Pediatric experience within the last 6 months Comfortable working both Peds (with a complex medical history) and Adults cases Experience with Pediatric

Cases: (ASA 3) Prematurity-Newborn Experience with Pediatric

Cases: (ASA 3) Newborn-1yr Experience with Pediatric

Cases: (ASA 3) 1-3 yr Experience with Pediatric

Cases: (ASA 3) 4-6yrs

• Type of Anes Model and supervision ratio: MEDICAL DIRECTION
• Age Range of Patients: 0-99; Locums do not typically do NICU cases under 6 months
• Electronic Medicine Records (EMR): EPIC
• Credentialing Timeframe: 60 Days
• Certifications/Qualifications Required (ACLS, BLS, MSQA, TEE, etc.): ACLS, PALS, Active CRNA/APRN license

• Must Be Board Certified
