Cases: Orthopedics, gen surg, gyn surg, ortho spine, endo, urology, GI, minor vascular, breast surgery. Peripheral Nerve Blocks: Yes


Facility: Large 720 Bed hospital, Level 1 Trauma - 14 ORs

Credentialing: 90 days
