Assignment: Must have Active AZ License

Length of Contract: Summer Coverage and Ongoing

Schedule: (5) 8-hour shifts (7AM-3PM), Monday - Friday Call,

Nights and Weekends: Call: 2 weeks no call; 3rd week float and take call.

Weekends: 4 week rotation

Cases: Mainly General and Ortho cases, some OB and occasional cath lab. OB: Yes Peripheral Nerve Blocks: Yes

Facility: Small 25-bed Critical Access Hospital with 2 OR s

Model: Independent CRNA Practice

Charting: Meditech

Credentialing: Temps in 30 days with a clean file
