1. CRNA is licensed and certified by the State of Kansas and will maintain a current active license. CRNA will provide Hospital a copy of his current license at each renewal date.
2. CRNA will apply for and maintain privileges on the professional medical staff of Hospital and comply with all medical staff by-laws, rules, and regulations.
3. CRNA will apply for and maintain professional liability insurance at his own expense with minimum coverage limits of $300,000/600,000. A certificate of insurance must be provided to the Hospital at each renewal period.
4. CRNA will provide anesthesia service in compliance with applicable statutes, rules and regulations and may participate in developing hospital policies and procedures relative to anesthesia services. CRNA acknowledges that medical necessity may require that he be available to provide services at any time of the day or night and CRNA agrees to be available in such instances.
5. Hospital will allow CRNA to provide anesthesia coverage at other area hospitals provided that such outside coverage does not conflict with CRNA's obligations to Hospital under this agreement.
6. CRNA will provide on-call anesthesia services for 13 weeks annually, 24 hours per day, and respond to Hospital within thirty (30) minutes of requests for services or as reasonably necessary.
7. CRNA will provide, at his own expense, all electronic communication devices (e.g., cellular telephone, pager) and their call numbers so Hospital can contact CRNA.
8. CRNA will not charge professional fees to Hospital related to anesthesia.
9. CRNA will provide complete medical record documentation and billing information on services rendered.
10. CRNA will provide services consistent with professional standards of practice.
11. CRNA will participate in Hospital's quality assurance program.
12. To the extent required by applicable law, the parties agree that, until the expiration of four years after the furnishing services provided pursuant to this agreement, the parties will make available upon request, to the Secretary of the United States Department of Health and Human Services and the United States Comptroller General and their duly authorized representatives, this agreement, and all books, documents and records necessary to certify the nature and extent of the cost claimed to Medicare for those services provided under this agreement. If any party carries out the duties of this agreement through a subcontract worth $10,000 or more over a twelve month period with a related organization, such contract shall also contain an access clause to permit access by the Secretary, Comptroller General and their duly authorized representatives, to the related organization's books and records.
13. CRNA will bill non-surgical pain procedures and clinic visits at the professional fees associated with them at a rate of $25.00 per point.
